A study conducted from October 2015 to April 2016 by a team from the University of Wisconsin School of Medicine and Public Health, in Madison, determined that 13% to 30% of hospital readmissions are due to poor transitions from the hospital to home. The team was able to work toward a solution by using the infrastructure of an existing phone-based transitional care program to meet the needs of surgical patients.
